Locations include:
Hampton, Newport News, Gloucester, Kilmarnock, and Southside, VA! Responsibilities as Speech Therapist (ST): Assists the physician in evaluating the patient's level of function. Assesses home environment and identifies equipment needs related to speech/language function. Identifies functional speech deficits and establishes goals and a plan of care to improve the patient's function, revising as necessary. Performs and completes recertifications, resumption-of-care, and discharge Comprehensive OASIS assessments and associated paperwork in accordance with agency policy. Instructs the patient and family members, as well as other members of the healthcare team, on methods to assist the patient in improving communication skills. Recommends communication devices/aids as indicated. Periodically re-evaluates and upgrades the patient's program to maximize functional improvement. Records and reports to the patient's physician the patient's treatment progress and any changes in the patient's condition and plan of care. Confers with the family, referring physician, and other agency professional staff to obtain information to promote coordinated, efficient, and quality patient care. Performs and submits required documentation of patients' evaluations, plan of care, clinical notes, progress reports, summaries, and changes in orders. All patient visit notes must be returned on the next scheduled workday, within forty-eight (48) hours. Maintains necessary statistical and billing data. Keeps abreast of changes in speech pathology practice and theory, contributes to the field through research, presentations, and in-services. Attends and contributes to in-services, clinical records reviews, and meetings required to ensure coordinated and comprehensive plans of care for the patients of the agency Qualifications for Speech Therapist (ST): A Master's Degree in Speech Pathology from a Department of Speech Pathology at an accredited college approved by the American Board of Examiners in Speech Pathology and Audiology of the American Speech and Hearing Association. Licensed in the State of practice by the State Board of Healing Arts to practice as a Speech Pathologist. Certificate of Clinical competence in speech pathology or audiology.
