Job Description

Part-Time Speech-Language Pathologist (SLP-CCC) Springfield Center for Living & Rehabilitation Springfield, Vermont Springfield Center for Living & Rehabilitation is seeking a dedicated and compassionate Part-Time Speech-Language Pathologist (SLP-CCC) to join our in-house rehabilitation team. This is an excellent opportunity to make a meaningful impact in a skilled nursing and long-term care setting while working alongside an experienced interdisciplinary team focused on exceptional resident outcomes. Position Highlights Part-Time Schedule In-House Rehabilitation Program Skilled Nursing Facility (SNF) and Long-Term Care (LTC) Setting Collaborative Interdisciplinary Team Environment Competitive Compensation Utilize Net Health and PointClickCare (PCC) documentation systems Responsibilities Evaluate, develop, and implement individualized treatment plans for residents with speech, language, cognitive-communication, and swallowing disorders Provide evidence-based therapeutic interventions to maximize functional outcomes and quality of life Collaborate with physicians, nursing staff, rehabilitation professionals, residents, and family members Complete timely and accurate documentation in Net Health and PCC Participate in care planning meetings and interdisciplinary team discussions Ensure compliance with all state, federal, and facility regulations Qualifications Master's Degree in Speech-Language Pathology Current Vermont Speech-Language Pathology license Certificate of Clinical Competence (CCC-SLP) required Skilled nursing, rehabilitation, or long-term care experience preferred Strong clinical, communication, and organizational skills Commitment to providing high-quality, resident-centered care Why Join Springfield Center for Living & Rehabilitation?
