Job Description

The Heart Failure Nurse Navigator will plan, develop, implement, and manage the Heart Failure Program under the direction of the Heart and Vascular Executive Director and Cardiology Medical Director. In addition to program development, the Heart Failure Nurse Navigator serves as a clinical liaison and advocate for patients with heart failure, ensuring seamless coordination of care across the continuum, from diagnosis through treatment and follow-up. This role involves patient education, care coordination, and support for both patients and their families, facilitating communication between multidisciplinary healthcare teams and helping patients navigate complex medical systems. Bachelor's Degree
Experience:
Three years of related experience. Requires working knowledge of specialized practices, equipment, and procedures. Licenses and Certifications
  • GEORGIA REGISTERED NURSE LICENSE OR MULTISTATE NURSING LICENSE
  • ACLS CERTIFICATION
  • HEALTHCARE PROVIDER
    (CPR) Qualification
  • Bachelor's degree in nursing required. Masters preferred.
  • Certification in Cardiac Nursing (e.g., CCRN, PCCN, CVRN) preferred.
  • Meets and maintains the qualifications and certifications outlined.
  • Excellent interpersonal and communication skills.
  • Strong organizational and problem-solving abilities.
  • Ability to work independently and collaboratively in a multidisciplinary setting.

  • Program Coordination:
  • Facilitates a team approach and collaborates with the multidisciplinary team to identify patients and follow them through the continuum of care.
  • Act as a liaison between patients, cardiologists, hospitalists, ED, primary care providers and support services and departments throughout Tanner Health.
  • Leads hospital-wide initiatives focused on reducing readmissions and improving cardiac outcomes. Identifies gaps and develops solutions.
  • Leads multidisciplinary team meetings for Heart Failure and attends Heart & Vascular Service Line Council.
  • Patient Coordination and Support:
  • Guide patients through the cardiovascular care continuum, including diagnosis, treatment planning, procedures, and post-treatment follow-up.
  • Provide emotional support, health coaching, and education tailored to individual patient needs.
  • Help patients understand treatment options, medications, and lifestyle changes.
  • Care Planning and Management:
  • Assist in developing and implementing individualized care plans.
  • Ensure timely scheduling of diagnostic tests, procedures, and follow-up appointments.
  • Monitor patient progress and intervene proactively in case of complications or non-adherence.
  • Education and Outreach:
  • Educate patients and families about heart health, disease prevention, and post-discharge care.
  • Assist with community outreach and education programs related to heart health.
  • Documentation and Compliance:
  • Accurately document all patient interactions in the electronic medical record (EMR).
  • Maintain compliance with HIPAA regulations and hospital policies.
  • Participate in quality improvement initiatives and data collection reporting for cardiac programs.
