Home Care/Transition Coordinator Vevay, IN 47043 $13

  • Part-time The incumbent of this position is responsible for the coordination of duties for the Independent Living Center with relation to the particular programs/services in which the Program Director/Executive Director assign.
The incumbent is also for maintaining a safe and healthy environment for consumers & clients, which includes the following duties: Job duties include but are not limited to: (The following statements are intended as general illustrations of the work in this job class and are not all inclusive for specific positions). Reports directly to Program Director Provides tasks which include:
  • Recruit, train, evaluate, and supervise staff members and volunteers.
  • Ensure all required and pertinent information/documentation is collected, tracked, correct, and entered into the data collection software for reporting and records are retained in accordance to regulation/requirement standards/policy/EDGAR/law.
  • Recruit staff, which may include screening, interviewing, advertising, coordinating and conducting background and reference checks when appropriate.
  • Ensuring all testing is accurately completed, updated, in compliance and current.
  • Ensuring all files are accurately kept according to regulation and policy and prepared for regular audits at all times.
  • Training which includes conducting orientation training, necessary follow-up trainings, compliance, health and safety standards, and ensuring all necessary and require documentation is collected and completed for all staff during the time of hire and also post-orientation including any which may or may not expire.
  • Recruitment of prospective consumers which includes outreach services, presentations, etc.
  • Clerical duties such as data entry, filing, making calls, returning calls, email, updating social media sites, website, efficiently operating office equipment including computer and organizing office and office files.
  • Reception duties such as answering the telephone, taking messages, greeting people and providing resource information when applicable.
  • Assist IL Program Director with daily operation of office.
  • Coordinate/attend Independent Living functions such as health fairs, Community Awareness functions such as local festivals and fairs.
  • Conduct consumer visits, complaint investigation, and problem resolution strategies while ensuring ALL necessary documentation is accurately completed, dated and filed for record.
  • Ensuring company policies, procedures, standards, and assurances are being complied in accordance to the direction provided by management.
  • Complete proper documentation/follow up for Independent Living Center's consumers including all Information Referrals.
  • Ensure the Notice of Authorizations are complied with, the Progress Notes are completed accurately and according to the NOA, develop Service Plans with clients, create schedules for PCA's and ensure compliance, field staff timesheets are completed accurately, required documentation is completed and filed appropriately, visit reports are developed and accurate, completion of all documents is accurate and all corrective actions plans are complied with efficiently.
  • Schedule, develop, coordinate, supervise, and report regarding Programs/Services.
  • Recruit new consumers, conduct consumer satisfaction reviews, reports, etc.
  • Collect data for all new consumers including completing proper documentation for each to open a file and enter information into system for proper compliance with procedures and regulations.
  • Keep office, break room, and restrooms clean, organized, and neat.
  • Assist with sending correspondence such as required mailing to different counties.
  • Complete forms and keep accurate records.
  • Other administrative duties including providing assistance to the general function of the office.
JOB DESCRIPTION
  • Identify, investigate, resolve, or attempt to resolve complaints made by or on behalf of IL recipients/consumers in regards to violations of their civil rights.
  • Retrieve and ensure all proper authorizations/documentation is received/completed for the records of the Independent Living Center.
  • Respond to requests for assistance on issues related to Independent Living including but not limited to the provision of services, lack of services, funding of IL programs, removal of obstacles and/or barriers, accessibility, access to voting, equal access, and more.
  • Follow-up, which may include phone calls, mailings, visits, etc. on consumers on a regular basis
  • Complete required documentation on consumers which includes, I & R's, report changes, Contact Reports, and complete all require data entry into Center programs, etc.
  • Actively participate as the Center representative in groups, councils, partnerships, committees, memberships, boards comprised of individuals with similar interests in Independent Living and the rights of individuals with disabilities of all ages.
  • Follow Program policies and procedures, direction, instruction, guidance, and assistance in advocating for IL recipients and/or consumers.
  • Identify systems programs affecting residents, represent the interests of consumers directly to government agencies, legislative committees, legislative members, media as requested by the CEO of SIILC only, analyze, comment on, provide public testimony and monitor the development and implementation of proposed or existing laws, regulations, policies, and actions that affect individuals with disabilities of all ages as requested by the SIILC, facilitate public comment, pursue administrative, legal and other remedies on behalf of consumers.
  • Recruit, train, supervise, and assure certification of volunteers in accordance with Program Policy.
  • Assist in the implementation and evaluation of the statewide programs, keep effective communications, and maintain current entry of information in the software.
  • Coordinate services to ensure successful transitions for consumers from facilities, employment services, school, and other necessities to optimize options and outcomes for those requiring transitions to maximize independent living.
  • Attend regular/required staff meetings, group meetings, fairs, appointments, etc.
  • Provide appropriate information to consumers as needed utilizing resources available
  • Increase community awareness and outreach within your service area.
  • Attend meetings of local organizations, advise individuals of SIIC's services.
  • Search, advise, and perform ways to increase awareness/outreach.
  • Perform other functions as necessary Essential Functions/Requirements This position may require employees to perform duties under varying conditions.
Ability to communicate courteously, effectively, and professionally with clients, outside contacts, and all levels of staff, volunteers, consumers, peers, etc. Ability to compose clear, concise, and accurate reports with minimal direct supervision. Be able to identify requirements/standards in the Rehab Act and ADA. Must be able to identify and quickly respond to potential problems and/or conflicts and react accordingly. Must have the ability to work independently and be self motivated. Must have reliable transportation, be willing to travel, and a valid drivers license and personal insurances. Must be able to safeguard extremely confidential information. This position is a part-time position offered through SIILC which may vary annually according to contractual agreement(s) and funding availability. SIILC reserves the right, at the discretion of the Director, to require a minimal and/or maximum number of hours/travel per pay period which may change periodically with minimal notice.
