Job Description
Job Summary Join our dedicated healthcare team as a Home Health Aide, where you will play a vital role in providing compassionate, personalized care to individuals in their own homes. This position offers an exciting opportunity to make a meaningful difference in patients' lives by assisting with daily activities, monitoring health status, and supporting overall well-being. As a Home Health Aide, you will work closely with clients, families, and healthcare professionals to deliver high-quality home and community care that promotes independence and dignity. Responsibilities Provide assistance with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility support Monitor and observe patients' health status, including vital signs and overall condition, documenting changes accurately in Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Assist with medication administration following prescribed protocols while maintaining compliance with HIPAA regulations for patient privacy and confidentiality Support patients with specialized needs including dementia care, Alzheimer's care, memory care, hospice care, and working with individuals with developmental disabilities or disabilities Operate medical equipment such as Hoyer lifts, catheters, and other assistive devices safely and effectively Prepare nutritious meals and assist with meal planning to meet dietary requirements; perform light cleaning and laundry to maintain a safe and comfortable environment Document medical records accurately, including patient observations, medication logs, and care activities; ensure compliance with healthcare standards Qualifications Proven experience in home health care or long-term care environments such as nursing homes or assisted living facilities Knowledge of medical terminology and familiarity with EMR/EHR systems for documentation purposes Certification in CPR (Cardiopulmonary Resuscitation) required; additional certifications such as medication administration or catheter care are a plus Experience working with individuals with disabilities or chronic conditions like dementia or Alzheimer's disease preferred Ability to perform patient observation accurately and respond promptly to changing health needs Strong communication skills to collaborate effectively with healthcare teams and families Valid driver's license and reliable transportation are often required for live-in employment or community-based visits Join us if you're passionate about delivering compassionate home health services!
