Home Health Aide

Duties Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, toileting, and mobility to enhance comfort and independence Monitor patient health by observing vital signs, noting changes in condition, and accurately documenting medical information using Electronic Health Record (EHR) systems Support medication administration following prescribed protocols while maintaining strict adherence to HIPAA regulations for patient privacy and confidentiality Provide specialized care including Alzheimer's care, dementia care, hospice support, catheter care, and memory care to meet diverse health needs Operate medical equipment such as Hoyer lifts and perform tasks like laundry, cleaning, meal preparation, and light housekeeping to maintain a safe and hygienic environment Collaborate with healthcare professionals by updating medical records, assisting with medical documentation, and communicating patient status effectively Engage with patients who have developmental disabilities or require long-term care by delivering compassionate home & community care tailored to individual goals Requirements Proven experience in home health or long-term care settings; nursing home experience is highly valued Knowledge of medical terminology, vital signs measurement, medication administration, and emergency procedures including CPR certification Familiarity with EMR (Electronic Medical Records) or EHR systems for accurate documentation Ability to assist with complex tasks such as catheter care, hospice support, dementia-related activities, and working with individuals with disabilities or developmental challenges Strong patient observation skills combined with a caring attitude; ability to work independently while maintaining excellent communication with team members Valid certification in CPR; training in Alzheimer's care or hospice preferred but not required Ability to perform physical tasks such as lifting using Hoyer lifts and assisting with mobility; flexibility for live-in employment is a plus Join us in delivering exceptional home health services that empower individuals to live safely and comfortably at home.
