Job Description

Job Summary Join our dedicated team as a Home Health Aide & Caregiver, where you will play a vital role in enhancing the quality of life for individuals in their homes and community settings. This position offers an exciting opportunity to provide compassionate, personalized care to diverse populations, including seniors, individuals with disabilities, and those receiving hospice or memory care. Your expertise will help clients maintain independence, dignity, and comfort while supporting their health and well-being. This paid position is ideal for motivated caregivers eager to make a meaningful difference every day. Responsibilities Deliver comprehensive patient care by assisting with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and mobility support using tools like Hoyer lifts and other assistive devices. Monitor and record vital signs including blood pressure, pulse, temperature, and respiration to ensure patient stability. Observe patients closely for changes in condition or behavior, documenting findings accurately in electronic health record (EHR) or medical documentation systems while maintaining HIPAA compliance. Administer medications and manage medication schedules following prescribed protocols to ensure safety and adherence. Provide specialized dementia and Alzheimer's care, including memory care techniques and tailored activities to promote cognitive engagement. Support patients with medical procedures such as catheter care, wound dressing changes, and hospice-related tasks with sensitivity and professionalism. Assist with household tasks like meal preparation, cleaning, laundry, and maintaining a safe environment to promote overall well-being. Collaborate effectively with healthcare teams using electronic medical records (EMR systems) to update patient information promptly and accurately. Work with individuals with developmental disabilities or disabilities by implementing personalized care plans that foster independence and dignity. Support clients in assisted living or long-term care settings by providing consistent, high-quality care aligned with their needs and preferences. Qualifications Prior experience in home health care, nursing homes, assisted living facilities or long-term care environments is highly preferred. Demonstrated ability to perform patient observation and respond appropriately to changing conditions. Knowledge of medical terminology, medication administration procedures, CPR certification, and HIPAA regulations. Experience working with individuals with dementia or Alzheimer's disease; familiarity with memory care strategies is a plus. Proficiency in using EMR/EHR systems for documentation; experience with medical records management is desirable. Ability to assist with ADLs using equipment such as Hoyer lifts; comfortable working with patients requiring catheter or hospice care. Strong communication skills combined with compassion for providing patient-centered care in home or community settings. Valid certifications in CPR; additional certifications related to hospice or dementia care are advantageous.
