This rewarding position involves assisting clients with daily activities, monitoring their health, and ensuring their comfort and safety. Your enthusiasm and attention to detail will make a meaningful difference in the lives of those you serve, fostering independence and dignity every day. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient health by observing vital signs and recognizing changes in condition; document medical information accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems Support medication administration following prescribed schedules and protocols while maintaining compliance with HIPAA regulations for patient privacy Provide specialized care such as Alzheimer's care, dementia care, memory care, hospice support, and working with individuals with developmental disabilities or disabilities Operate medical equipment including Hoyer lifts, catheters, and other assistive devices; perform catheter care as needed Assist with meal preparation, feeding, light cleaning, laundry, and maintaining a safe and clean environment for clients Document medical records thoroughly and communicate effectively with healthcare teams regarding patient status and care plans Requirements Proven experience in home health caregiving or long-term care environments such as nursing homes or assisted living facilities Knowledge of medical terminology, vital signs measurement, medication administration, and medical documentation practices Certifications such as CPR (Cardiopulmonary Resuscitation) are required; additional certifications like hospice care or dementia training are a plus Ability to work independently and demonstrate patience, compassion, and excellent communication skills Experience working with individuals with disabilities or special needs preferred; familiarity with EMR/EHR systems is advantageous Physical ability to lift patients using assistive devices like Hoyer lifts; capable of performing patient observation tasks over extended periods Commitment to maintaining patient confidentiality in accordance with HIPAA standards This position offers an enriching opportunity to make a positive impact through dedicated caregiving.
