In this vital role, you will provide essential support to clients by assisting with daily activities, monitoring health status, and ensuring a safe, comfortable environment. Your energetic approach and attention to detail will help promote independence and enhance the quality of life for those you serve. This paid position offers an opportunity to grow your caregiving expertise while making a positive impact in the community. Responsibilities Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, toileting, and mobility support Monitor patient observations such as vital signs and overall health status, reporting any concerns promptly Support medication administration following proper protocols and documentation procedures Utilize Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems to accurately record patient information and care activities Provide specialized care for individuals with developmental disabilities . Assist with medical procedures such as catheter care, Hoyer lift transfers, and hospice support when needed Perform light housekeeping tasks like cleaning, laundry, meal preparation, and maintaining a safe environment in compliance with HIPAA regulations Requirements Proven experience in home health care or long-term care environments in a group home setting; experience in assisted living or nursing homes is highly valued Knowledge of medical terminology and caregiving best practices for patients with disabilities or chronic conditions Certification in CPR (Cardiopulmonary Resuscitation) and first aid required Ability to work independently while following detailed care plans and medical documentation standards Strong patient observation skills to identify changes in condition or behavior promptly Familiarity with EMR/EHR systems for accurate record keeping Compassionate attitude with excellent communication skills to work effectively with clients and their families Valid driver's license and reliable transportation preferred for live-in or community-based assignments Must be able to speak and write english Join us to provide compassionate home health services that truly make a difference!
