Job Description

Seasons Change Home Health Care, Inc. is seeking compassionate and dependable Home Health Aides to join our growing care team. This position plays an essential role in supporting patients with their daily living needs, while ensuring comfort, dignity, and safety within the home. Our aides are the heart of our care, helping individuals remain in the place they love most — their homes. Seasons Change Home Health Care, Inc. is a Medicare Certified Home Health Agency, Hospice, and Private Duty Agency located in Pigeon, Michigan. Serving 18 counties across the Greater Mid-Michigan Area and beyond, we have been proudly providing care for over 27 years. Accredited by CHAP and family-owned and operated, our mission is to deliver professional, compassionate, and personalized care to every patient we serve. Responsibilities Provide personal care assistance such as bathing, grooming, dressing, and mobility support. Assist patients with daily living activities to promote comfort and independence. Prepare and plan nutritious meals in accordance with patient needs. Monitor and report changes in patient condition to the supervising nurse. Support a safe and healthy home environment, including light housekeeping and laundry. Adhere to agency policies, care procedures, and professional standards of conduct. Uphold the highest level of compassion, dignity, and respect in all care provided. Key Qualifications High school diploma or GED required. Valid driver's license and reliable transportation. Compassionate, honest, dependable, and patient-centered. Ability to follow a care plan and communicate effectively with the care team. Previous caregiving or healthcare experience preferred, but not required — training provided.
