Home Health Aide:

• $15.00/hr - Multiple shifts/schedules available/weekend availability needed

Responsibilities:

Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, and toileting Provide support with mobility, transfers, and ambulation Meal preparation in accordance with patient needs Provide medication reminders (no administration) Perform light housekeeping duties Monitor and report changes in patient condition to the clinical team Provide companionship and emotional support to patients Follow individualized care plans and agency policies Tasks performed by a Home Health Aide must be assigned and directed under the supervision of a registered nurse. Documents care services rendered on daily progress notes and submits them to the Agency weekly. Reports any change in the client's condition or in his or her home situation to the staff nurse or Director of Nursing Flexibility to support patients in

Scioto County, Piketon and Ironton Qualifications:

Previous home care or healthcare experience preferred Reliable transportation and valid driver's license/insurance Ability to pass background check Strong communication and interpersonal skills Compassionate, dependable, and patient-focused HHA or STNA certification is preferred; training is available.

Organizational Relationship:

• Reports directly to the staff nurse and indirectly to the Director of Nursing.

Functional Abilities:

• Must be able to communicate verbally and in writing in a manner understood by most persons.
• Must be able to lift and transfer clients.
• Must be able to travel to client's residences.
