Essential Functions The Home Health Aide is responsible for providing patients with in-home personal care and designated health related services to maintain the patient's physical and emotional wellbeing, while following the written plan of care, Medicare/Medicaid regulations and agency policies and procedures. Understand and adhere to established agency policies and procedures Perform personal hygiene and hands-on care as assigned Complete appropriate visit records in a timely manner as per agency policy Reports changes in the patient's condition and needs to the RN Performs household services essential to health care in the home environment Ambulates and exercises the patient as needed Perform simple procedures as an extension of the therapy or nursing services Assists with medications that are ordinarily self-administered as needed Attends in-service and continuing education programs as scheduled as necessary Job Qualifications and Requirements Must meet Medicare Conditions of Participation for Home Health Aide training program and competency Have a sympathetic attitude toward the care of the sick and elderly Ability to carry out directions with exceptional communications skills The maturity and ability to deal effectively with the demands of a home care position License Requirements Current driver's license and vehicle insurance, access to a dependable vehicle or public transportation.
