What You Will Do as a Hospice Social Worker Have a passion for serving others through love? As a Stonerise Social Worker, you will be responsible for assessing the psychological status as it relates to the patient's illness and environment, carrying out social evaluations, and providing information and services to the patient and families. This is start-up branch with a chance to make this position full-time. Some of your primary responsibilities include: Assessing the psychological status of patients related to the patient's illness and environment and communicating findings to the registered nurse and IDT. Carrying out social evaluations and planning interventions based on evaluation findings. Maintaining clinical records for all patients referred to social work. Serving as a liaison between patients or families and community agencies. Participating in the development of the total plan of care and case conferences as required. Assisting the physician and other team members in understanding significant social and emotional factors related to health problems. Assisting members of interdisciplinary teams in understanding the significance of social and emotional factors that relate to the patient's care. Assessing and reassessing social and emotional factors in order to help the patient and family cope with problems in daily living and to help them follow medical recommendations to maximize the patient's comfort. Counseling patients and families concerning the facilitation of the medical plan of treatment and end-of-life care. Participating in the agency's QAPI program. Ensuring compliance with the state and federal regulations, company policies/procedures, and process maps in the performance of job duties. Other duties as delegated by the Home Health and Hospice Administrators. Possesses and maintains current CPR certification. Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order. At least 1 year of social work experience in a healthcare setting preferred. Possesses a current Medical Social Worker Licensure in the states of Ohio.
