JOB OVERVIEW
We are seeking a dynamic and compassionate Home Health Clinical Manager (RN Required) to lead our dedicated team in delivering exceptional patient care within the home health setting. This vital role combines clinical expertise with leadership skills to ensure high-quality, compliant, and patient-centered services. As a Clinical Manager, you will oversee clinical operations, coordinate interdisciplinary teams, and foster a positive environment that promotes continuous improvement and excellent patient outcomes. Your expertise in diverse nursing disciplines and ability to manage complex cases will drive success in this fast-paced, rewarding environment. DUTIES Lead and supervise nursing staff, including nurse management and team coordination, ensuring adherence to clinical protocols and regulatory standards such as HIPAA and JACHO standards. Oversee patient assessments, medical documentation, and care planning for individuals with diverse needs including geriatrics, behavioral health, dementia care, pediatric care, and acute pain management. Coordinate clinical workflows utilizing EMR systems to ensure accurate documentation of vital signs, medical records, medication administration, and diagnostic evaluations. Manage utilization review and utilization management processes to optimize patient care plans while maintaining compliance with DRG coding (Diagnosis-Related Group) and CPT coding standards. Conduct regular audits of medical records, ensure proper
ICD-9/ICD-10
coding practices, and oversee discharge planning to facilitate smooth transitions of care. Provide training on sterile techniques, infection control practices, and wound care. Ensure staff compliance with OSHA standards and infection control protocols while fostering a culture of safety and quality improvement. SKILLS Demonstrated ability to supervise and direct professional administrative personnel. Ability to market and deal tactfully with customers and community. Has excellent observation, verbal and written communication skills. Knowledge of business management, governmental regulations and the joint commission standards.
WHAT YOU BRING
Active California RN license required, no exceptions Minimum 3 years of home health clinical experience required Minimum 3 years of clinical management experience required Strong OASIS and Medicare home health regulatory knowledge Experience with home health EMR systems (HomeCare HomeBase, Axxess, or similar) Proven ability to lead a team and improve patient outcomes Strong communication, organizational, and problem-solving skills
