Home Health Physical Therapist

Home Health Physical Therapist | Full-time $60-70 per visit, usually around 25-30 visits per week We are seeking a dedicated full-time Physical Therapist to join our team. The ideal candidate will provide exceptional patient care in various home settings! This is an excellent opportunity for a skilled Physical Therapist looking to make a positive impact on patients' lives across various healthcare settings.

Responsibilities:

• Conduct patient assessments and develop individualized treatment plans
• Administer physical therapy treatments to patients
• Educate patients and their families on treatment plans and exercises
• Monitor and document patient progress
• Collaborate with healthcare professionals for comprehensive patient care
• Stay updated on advancements in physical therapy practices

Requirements:

• Bachelor's or Master's degree in Physical Therapy
• State licensure as a Physical Therapist
• Profound knowledge of anatomy and physiology
• Experience in acute care, outpatient settings, hospital medicine, orthopedics, or clinics
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
