Job Summary Join our dedicated team as a Part-Time Home Health Physical Therapist (PT) and make a meaningful difference in patients' lives through flexible, personalized care. In this role, you will deliver expert physiotherapy services to patients in their homes and community settings, promoting recovery, independence, and overall well-being. Your expertise will help develop comprehensive care plans, facilitate discharge planning, and ensure seamless coordination with healthcare teams to optimize patient outcomes. Duties Conduct thorough assessments of patients' physical conditions in home and community environments, utilizing physiology and anatomy knowledge to inform treatment strategies Develop and implement personalized care plans tailored to individual needs, including therapy techniques such as trigger point therapy and orthopedic interventions Provide patient-centered care across inpatient hospital settings, outpatient clinics, and home health visits, ensuring safe and effective therapy delivery Assist with discharge planning by evaluating patient progress and coordinating follow-up care or referrals as needed Document all patient interactions accurately within electronic medical record (EMR) systems, maintaining compliance with healthcare regulations Educate patients and caregivers on exercises, mobility techniques, and injury prevention strategies to promote long-term health benefits Collaborate with multidisciplinary teams to ensure continuity of care and optimize treatment outcomes for diverse populations Requirements Valid state licensure as a licensed Physical Therapist (PT) with current registration in good standing Proven experience in home health settings or community-based physiotherapy preferred; inpatient hospital or outpatient clinic experience is highly valued Strong knowledge of medical terminology, physiology, anatomy, and various therapeutic techniques including trigger point therapy and orthopedics Demonstrated ability to develop effective care plans for pediatric, geriatric, orthopedic, and post-surgical patients Familiarity with EMR systems for documentation and patient record management Excellent communication skills to educate patients and coordinate with healthcare teams effectively Ability to adapt quickly to different environments such as hospitals, clinics, or patients' homes while maintaining high standards of patient care Join us in delivering compassionate physiotherapy services that empower individuals to regain their independence!
