Job Summary We are seeking a dedicated and compassionate Home Health Registered Nurse to join our dynamic healthcare team. Responsibilities Perform comprehensive patient assessments, including vital signs, physical examinations, and review of medical histories to develop individualized care plans. Administer medications and treatments such as IV infusions, injections, wound suturing, and specialized therapies like botulinum toxin treatments. Conduct skilled procedures including catheterizations, tube feedings, sterile dressing changes, and ostomy care following aseptic techniques. Monitor patient progress through ongoing evaluations and adjust care plans accordingly; communicate findings with physicians and interdisciplinary teams. Educate patients and their families on disease management, medication administration, mobility assistance, and health coaching strategies to promote independence. Maintain accurate medical documentation within electronic health record (EHR) systems such as Epic or Cerner while adhering to HIPAA regulations. Coordinate discharge planning, case management activities, and collaborate with other healthcare providers to ensure seamless continuity of care across settings. Skills Proven experience in home health nursing with familiarity in medical-surgical, geriatrics, or critical care environments. Strong knowledge of physiology, anatomy, pharmacology, and diagnostic evaluation techniques including ICD coding (ICD-10/ICD-9). Proficiency with EMR/EHR systems such as Epic or Cerner for accurate documentation and patient record management. Skilled in performing complex procedures like IV insertions, phlebotomy, laboratory specimen collection/processing, and airway management. Experience working with individuals with disabilities or developmental delays; familiarity with memory care and dementia/Alzheimer's care is highly valued. Ability to manage acute pain cases effectively while demonstrating excellent triage skills in urgent or emergency situations. Knowledge of infection control practices and sterile processing standards to ensure patient safety. Strong communication skills for patient education, team collaboration, and adherence to NCQA standards for quality care delivery.
