Job Description
Job Overview Join our dynamic healthcare team as a Home Health Registered Nurse, where you will play a vital role in delivering compassionate, comprehensive nursing care directly to patients in their homes. This energetic position offers the opportunity to empower individuals through personalized health management, ensuring safe and effective treatment plans. As a key member of our home health services, you will utilize your clinical expertise to assess patient needs, coordinate care, and promote optimal health outcomes in a variety of community settings. Responsibilities Conduct thorough patient assessments, including vital signs, physical examinations, and detailed health histories to determine individual care needs. Administer medications via injections, IV infusions, and other routes while adhering to aseptic techniques and safety protocols. Manage complex medical procedures such as wound suturing, ostomy care, and ventilator management with precision and confidence. Provide specialized care for diverse populations including geriatrics, pediatrics, individuals with developmental disabilities, and those requiring hospice or palliative services. Collaborate with interdisciplinary teams using electronic health record (EHR) systems like Epic or Cerner to document patient progress accurately and efficiently. Educate patients and their families on disease management, medication administration, nutrition, and self-care strategies to foster independence. Coordinate discharge planning and case management efforts to ensure seamless transitions between hospital, outpatient, and home settings. Qualifications Valid Registered Nurse (RN) license with current state certification. Proven experience in home health nursing or related fields such as medical-surgical, critical care, ICU, or emergency medicine. Familiarity with EMR/EHR systems like Epic or Cerner; experience with documentation standards such as NCQA is a plus. Strong knowledge of medical terminology, physiology, ICD coding (ICD-10/ICD-9), and diagnostic evaluation techniques. Skilled in performing invasive procedures including phlebotomy, venipuncture, catheterization, and sterile dressing changes. Ability to work independently in patients' homes while maintaining high standards of infection control and HIPAA compliance. Excellent patient assessment skills combined with compassionate communication tailored to diverse populations including seniors and children. Critical care experience preferred—especially ICU or trauma center background—and familiarity with complex treatments such as dialysis or cardiac catheterization is advantageous. Additional certifications such as Basic Life Support (BLS), Advanced Cardiovascular Life Support (ACLS), or specialized training in hospice & palliative medicine enhance candidacy.
