Job Description

Overview Join our dynamic healthcare team as a Home Health Registered Nurse, where your expertise will directly impact patient recovery and well-being in the comfort of their own homes. This vital role offers an exciting opportunity to deliver personalized, compassionate care while utilizing your diverse clinical skills. As a key member of our home health services, you will coordinate patient assessments, administer treatments, and educate families to promote health and independence. Responsibilities Conduct comprehensive patient assessments including vital signs, physical examinations, and medical history reviews to determine care needs. Administer medications, injections, IV infusions, and perform wound care such as suturing and ostomy management following aseptic techniques. Manage complex care plans involving chronic conditions like diabetes, heart disease, respiratory issues, and neurological disorders. Utilize electronic health record (EHR) system, Axxess, to document patient interactions accurately and maintain detailed medical records. Educate patients and their families on medication administration, disease management, mobility assistance, nutrition, and safety measures at home. Coordinate with multidisciplinary teams including physicians, therapists, social workers, and case managers to ensure seamless care delivery. Provide triage support by assessing urgent needs over the phone or during visits and escalating cases appropriately for emergency or specialized intervention. Experience Valid RN license with current state registration or licensure required. Proven experience in home health nursing or related settings such as hospitals, nursing homes, assisted living facilities, or outpatient clinics preferred. Familiarity with EMR/EHR systems like Axxess to ensure efficient documentation and communication. Critical care experience including ICU or emergency medicine is highly desirable for managing acute situations. Knowledge of specialized areas such as hospice & palliative medicine, infectious disease care, trauma medicine (Level I & II trauma centers), dialysis support, cardiac catheterization procedures, and long-term care practices. Skills in IV insertion, medication administration via various routes, specimen collection/processing, and infection control protocols. Experience working with diverse populations including pediatrics (neonatal ventilator), geriatrics (memory & dementia care), behavioral health patients, individuals with developmental disabilities or disabilities requiring complex case management.
