Our mission is to support each patient's healing journey through communication, motivation, and personalized care. Position Summary We're seeking a Registered Nurse (RN) to join our clinical team. The RN is responsible for the delivery and coordination of patient care services, ensuring that the Plan of Care is implemented effectively and in compliance with state and federal regulations. This position offers flexible scheduling and the opportunity to make a meaningful difference in patients' lives in the comfort of their own homes. Responsibilities Perform initial evaluations, recertifications, resumptions of care, and discharges. Develop and implement individualized Plans of Care in collaboration with physicians and interdisciplinary team members. Supervise LVNs, HHAs, and other staff assigned to the patient's care. Conduct in-home supervisory visits at least every 14 days for patients receiving aide services. Monitor and update patient medication records and treatment documentation. Communicate effectively with patients, families, physicians, and office staff. Ensure timely and accurate submission of clinical documentation. Participate in case conferences, staff education, and quality improvement activities. Provide on-call support as needed and represent the agency in a professional manner.
Qualifications Required:
Current California RN license Reliable transportation and ability to travel to patient homes Ability to lift, transfer, and assist patients as needed Strong interpersonal and communication skills
Preferred:
Prior experience in home health, hospital, or community care setting Bilingual (any language) IVIG experience Required Credentials (must remain current): RN License Driver's License & Auto Insurance TB Test /
Chest X-Ray Physical Exam CPR Certification Pay:
$80,000.00
