This position offers a unique blend of autonomy, patient-centered care, and strong organizational support within a mission-driven environment.

$57/hourPermanent Salary:
$41-$48/hour (based on experience) Role Summary The Registered Nurse (RN) will manage a caseload of residents requiring nursing care and support services in a community-based setting. This role includes conducting telehealth intakes, ongoing assessments, and care coordination in collaboration with an interdisciplinary team. You will play a key role in delivering high-quality care that promotes independence, safety, and overall well-being for senior residents. Key Responsibilities Provide nursing care and education to residents and family/caregivers Assess patient needs, monitor progress, and update care plans accordingly Manage lab orders, track results, and coordinate follow-up care with physicians Respond to emergency and non-emergency calls, including PERS alerts Collaborate with interdisciplinary team members to support resident needs Complete documentation accurately and in a timely manner Support telehealth visits, assessments, and ongoing care coordination Perform skilled nursing visits (as needed) for home health services Participate in emergency preparedness activities and drills Maintain compliance with clinical, safety, and regulatory standards Qualifications Active Registered Nurse (RN) license in Massachusetts (in good standing) Bachelor's degree in Nursing (preferred) Minimum 2 years of RN experience (home health experience preferred) Strong clinical judgment and ability to work independently Proficiency with electronic medical records and basic computer systems Excellent communication and interpersonal skills Core Competencies Patient-focused, compassionate care approach Ability to work independently with minimal supervision Strong organizational and time management skills Professional communication across diverse populations Knowledge of HIPAA compliance and patient confidentiality Physical Requirements Ability to move throughout the workday and travel within the community Frequent standing, walking, reaching, and use of hands Ability to lift up to 50 pounds Must meet health and safety requirements per organizational policy Additional Requirements Background check required Compliance with OSHA and all applicable health and safety regulations Why Work with This Organization?
