Home Health Visiting RN (PRN/Part-time)

Job Summary Meridian Home Health Care Services, Inc, seeking a dedicated and compassionate Registered Nurse (RN) for our Home Health agency. The ideal candidate will provide high-quality nursing care to patients in their homes, ensuring their comfort and well-being while promoting recovery and independence. This role requires strong clinical skills, excellent communication abilities, and a passion for patient-centered care. Duties Conduct comprehensive assessments of patients' health status and needs in their home environment. Develop and implement individualized care plans based on patient assessments and medical history. Administer medications and treatments as prescribed, ensuring adherence to safety protocols. Monitor patients' progress, documenting changes in condition and response to treatment. Provide education to patients and their families regarding health management, disease prevention, and medication administration. Collaborate with interdisciplinary teams to coordinate care, including discharge planning and referrals to other services as needed. Maintain accurate medical records and documentation in compliance with legal and regulatory standards. Respond promptly to patient inquiries or concerns, providing support and guidance as necessary. Requirements Current Registered Nurse (RN) license in the state of practice. Experience in acute care settings such as ICU or ER is preferred. Familiarity with pediatrics, behavioral health, infectious disease care, or discharge planning is a plus. Strong knowledge of medical terminology and ability to interpret clinical data effectively. Excellent communication skills with the ability to build rapport with patients and families. Ability to work independently while managing time effectively in a home health setting.
