As the Homecare Referral Coordinator, you will be a vital member of a dynamic team responsible for… Under the supervision of DKH At Home clinical leadership, the Homecare referral coordinator is responsible for processing and coordinating home health and hospice referrals and supporting patients throughout the transition from referral to initiation of services. This position serves as a central communication and customer service resource for referral sources, providers, patients, families, and internal staff. Responsibilities include collection and entry of referral, demographic, insurance, and physician information; coordination of required documentation and authorizations; monitoring patient transition status across healthcare settings; and collaboration with clinical managers and operational staff to support timely initiation of services. The Homecare referral coordinator monitors referred patients for hospitalizations, facility admissions, discharge readiness, and changes in status to facilitate continuity of care and effective coordination of services. This role functions in an administrative and coordination capacity and does not provide direct patient care or independent clinical assessment.Homecare Referral Coordinator Responsibilities:
Coordinates and processes home health and hospice referrals from referral through transition to service. Provides exceptional customer service and communication to referral sources, providers, patients, families, and internal staff. Maintains accurate referral, insurance, physician, and transition-of-care documentation within the electronic medical record. Collaborates with clinical managers, scheduling staff, billing, hospitals, and referral sources to support timely initiation of services. Monitors referral and patient transition status, including hospitalizations, facility admissions, and discharge readiness, to facilitate continuity of care and timely start of home health or hospice services. Supports regulatory compliance through accurate documentation management and coordination of required referral processes.Homecare Referral Coordinator Education and Experience Requirements:
Education High School diploma or equivalent required. Successful completion of a Medical Assistant program or similar healthcare support training preferred Medical terminology coursework preferred. Experience One to three years of experience in a healthcare office, referral, scheduling, referral coordination, customer service, or related healthcare support role preferred. Experience with electronic medical records and healthcare documentation preferred. Experience in home health, hospice, hospital discharge planning, insurance authorization, or care coordination preferred. Experience managing multiple priorities in a fast-paced healthcare environment preferred.Homecare Referral Coordinator Knowledge, Skills, and Abilities:
Exceptional customer service and interpersonal communication skills. Strong organizational skills and attention to detail. Ability to prioritize and manage multiple referrals, calls, and requests simultaneously. Ability to communicate professionally and compassionately with patients, families, providers, referral sources, and interdisciplinary staff. Strong computer skills, including proficiency with electronic medical records, Microsoft Office applications, and various web-based platforms. Ability to work independently and collaboratively within a team environment. Ability to maintain professionalism and composure in high-pressure or emotionally sensitive situations. Knowledge of medical terminology and healthcare documentation practices preferred.
