Salary:
Starting at $40.00 Job description: The Admissions Nurse is responsible for performing the initial admission, patient/caregiver needs assessments, and initiating the hospice plan of care and accompanying services between the time of admission and transition to a case manager nurse, as well as performing other general hospice nurse duties as may be required or assigned. Essential functions include, but are not limited to:
  • Perform initial admission patient/caregiver needs assessments including identifying physical, psychosocial, cultural, and spiritual implications that may influence patient/caregiver
  • Determine patient's appropriateness for hospice services, routine or hospice general inpatient, consistent with OCH admission criteria policies and in conjunction with the patient's MD
  • Initiate and manage the hospice plan of care and accompanying services between the time of admission and transition to primary nurse, including collaboration with the interdisciplinary team and patient's primary MD
  • Educate, review, evaluate and document the understanding and acceptance of the patient/caregiver in regards to all aspects of hospice standards and care to be expected
  • Schedule assessment/information visits with potential hospice patients and their caregivers as requested by the Clinical Managers
  • Provide specialized pain/symptom management related to end of life discomfort
  • Provide specialized hospice training to family and/or to facility staff/contracted employees to ensure adequate care as may be required
  • Share in providing on-call/continuous care coverage as may be needed, completing necessary documentation
  • Completes timely informational meetings, assessments, and all paperwork/follow-up of patients referred; documentation must substantiate and justify hospice services and comply with state/federal regulations and accrediting body standards
Qualifications:
  • Licensed to practice as a Registered Nurse in the Commonwealth of Massachusetts (BS preferred)
  • Ability to use good judgment and discretion and to work effectively as a member or part of an interdisciplinary team
  • Must have knowledge and understanding of applicable federal, state, and local hospice, Medicare, Medicaid, HMO regulations, as well as HIPAA and other regulations protecting personal health information of patients
