Hospice Agency Coordinator Job Description Position Summary The Hospice Agency Coordinator is responsible for overseeing the daily administrative and operational coordination of hospice services. This role ensures timely patient admissions, effective communication between interdisciplinary team members, regulatory compliance, and efficient scheduling of services. The Coordinator serves as a central point of contact to support clinical staff, patients, families, and referral sources. Key Responsibilities1. Intake & Admissions Coordination Receive and process hospice referrals from hospitals, physicians, and facilities Verify patient eligibility, insurance coverage, and required documentation Coordinate admission visits with hospice nurses and physicians Ensure all admission paperwork is completed accurately and timely 2. Scheduling & Staffing Coordinate schedules for nurses, aides, social workers, and other hospice staff Ensure timely coverage for patient visits and on-call services Adjust staffing based on patient census and acuity 3. Interdisciplinary Team Support Facilitate communication between clinical team members (RN, social worker, chaplain, etc.) Coordinate interdisciplinary group (IDG) meetings and maintain records Track care plan updates and ensure timely documentation 4. Compliance & Documentation Maintain patient records in accordance with federal and state hospice regulations Ensure documentation meets standards required by Medicare hospice Conditions of Participation Assist with audits, surveys, and quality assurance initiatives 5. Communication & Customer Service Serve as a primary point of contact for patients, families, and referral sources Respond to inquiries and resolve issues promptly and professionally Maintain strong relationships with hospitals, physicians, and community partners 6. Billing & Administrative Support Assist billing team with verification, authorizations, and documentation tracking Monitor census, admissions, discharges, and transfers Support payroll inputs, visit tracking, and operational reporting
