Job Description

Schedule:
FT, PT, or PRN Compensation:
Starting compensation range of $25.00 - $28.00 hourly. Exact compensation will be determined by experience, education, licensure/certifications, and location, in accordance with applicable laws.

Position Overview:
The Hospice Aide is a compassionate, skilled member of Loma Linda University Hospice's interdisciplinary care team, providing essential personal care and supportive services to patients in the final stages of life. Working under the supervision of a Registered Nurse (RN) and in accordance with each patient's individualized Plan of Care (POC), the Hospice Aide assists patients with activities of daily living, comfort measures, and personal hygiene — meeting them wherever they call home, whether that's a private residence, skilled nursing facility (SNF), assisted living facility (ALF), or residential care setting. This role is often the most consistent human presence in a patient's final days. The Hospice Aide is not just a care provider — they are a trusted companion, a steady hand, and a meaningful source of comfort for patients and families navigating one of life's most profound experiences.
Key Outcomes & Responsibilities:
Maximize Patient Comfort and Dignity:
Deliver compassionate and complete personal care (bathing, oral hygiene, hair care, linen changes, dressing/undressing) following the Plan of Care (POC) to ensure the patient's comfort and dignity.
Ensure Patient Safety:
Proactively identify and eliminate safety hazards, properly utilize safety equipment (e.g., side rails, oxygen), and execute safe patient transfers, positioning, and ambulation as directed to prevent injury. Support Activities of Daily Living (ADLs): Assist patients with essential ADLs, including feeding and timely toileting (bedpans/urinals), to maintain comfort and continence.
Monitor and Report Clinical Status:
Accurately measure and record vital signs and skin condition; immediately assess and report any significant changes in the patient's physical or emotional condition to the Case Manager to ensure timely clinical intervention.
Support Therapy and Mobility:
Execute directed basic therapeutic procedures (e.g., range of motion exercises) to support the patient's mobility, comfort, and prevent complications.
Maintain Compliance and Quality:
Strictly adhere to all state regulations, organizational documentation standards, care protocols, and professional conduct policies; actively contribute to the maintenance of a safe, clean patient environment and participate in Quality Assessment Performance Improvement (QAPI) activities.
Cultivate Therapeutic Relationships:
Employ respectful and confidential communication techniques to foster trust and honor the home environment and personal needs of patients, families, and caregivers.
Requirements:
Completion of a state-approved Home Health Aide (HHA) training program, or equivalent hospice aide training that meets CMS Hospice Conditions of Participation Minimum 1 year of experience as a CNA or HHA; hospice, home health, SNF, or palliative care experience strongly preferred Current CPR/BLS certification Valid California driver's license, reliable personal vehicle, and proof of current auto insurance Ability to work independently in a field-based environment with strong time management skills Basic proficiency with mobile devices and electronic documentation tools Ability to read, write, and communicate effectively
Preferred Qualifications:
Certified Nursing Assistant (CNA) preferred
Bilingual Work Environment & Physical Requirements:
Sustain frequent physical activity including standing, walking, bending, stooping, kneeling, and reaching across diverse patient settings (homes, facilities). Safely transfer, reposition, and move patients and equipment, requiring the ability to lift or assist up to 50 pounds (with support/equipment for heavier transfers). Maintain the dexterity, visual, and auditory capacity necessary to perform clinical tasks, assess patient safety, and communicate effectively. Ability to respond promptly and perform CPR in urgent or emergency situations. Reliable access to and operation of transportation for consistent travel within the service area. Comfort working in a range of home environments, including exposure to pets, smoke, and varying indoor/outdoor conditions. Proficiency in using electronic systems (computers, tablets) for accurate and timely clinical documentation and communication. Ability to manage the unique emotional demands inherent in providing compassionate end-of-life care.
