Per Diem At Andwell Health Partners , we believe hospice care is about more than comfort—it's about connection, dignity, and honoring each person's journey. We are seeking a compassionate and experienced Hospice Chaplain to provide spiritual and emotional support to patients and families facing life-limiting illness.
Position Summary:
As a vital member of our interdisciplinary hospice team, the Hospice Chaplain offers non-denominational spiritual care, grief support, and guidance tailored to each patient's beliefs, values, and cultural background. This role is central to delivering holistic, person-centered care in homes, long-term care facilities, and other settings where patients reside.
Key Responsibilities:
Provide spiritual counseling, comfort, and presence to patients and families Support individuals of all faiths, spiritual paths, or no religious affiliation Facilitate life review, legacy work, and end-of-life rituals as appropriate Collaborate closely with hospice nurses, social workers, aides, and volunteers Assist with bereavement support and family follow-up after death Document visits and assessments in the electronic health record Participate in interdisciplinary team meetings and care planning
Qualifications:
Master's degree in Divinity, Theology, Pastoral Counseling, or related field Ordination or commissioning by a recognized religious body preferred Minimum 1 year of experience in hospice, healthcare chaplaincy, or spiritual care Clinical Pastoral Education (CPE) units strongly preferred Strong interpersonal and listening skills with a compassionate, nonjudgmental approach Valid driver's license and reliable transportation Why Join Andwell Health Partners?
