• Full-time Three Rivers Hospice is seeking a compassionate and dependable Licensed Practical Nurse (LPN) Case Manager to join our team in Mount Vernon.

What You'll Do Provide in-home and facility-based nursing visits to hospice patients Monitor patient conditions and communicate changes to the RN Case Manager and care team Assist in carrying out the plan of care focused on comfort and quality of life Administer medications and treatments as ordered Provide education, guidance, and emotional support to patients and families Complete documentation accurately and in a timely manner Collaborate with the interdisciplinary hospice team to ensure excellent patient care What We're Looking For Active Missouri LPN license Previous nursing experience required (hospice or home health preferred) Strong communication skills and compassionate bedside manner Ability to work independently and manage your daily schedule Reliable transportation, valid driver's license, and current auto insurance Why Join Three Rivers Hospice?
