Job Description

A facility near Sparta, WI is seeking a Hospice Registered Nurse to join their hospice and palliative care team! Hospice Registered Nurse Opportunity Full Time Permanent employment
Schedule:
Day Shift; Mon-Fri with weekend and holiday rotation Collaborates with the hospice interdisciplinary team to develop and implement a comprehensive plan of care based on patient, family, and team assessments. Completes functional assessments and initiates the plan of care to identify needs, manage symptoms, and establish patient/family goals. Provides direct nursing care, supervises care staff and volunteers, and evaluates progress using strong clinical and communication skills.
Hospice Registered Nurse Qualifications:
Unrestricted RN license with the ability to practice in Wisconsin Must have or be willing to obtain BLS and CHPN certifications Previous Nursing experience required Experience in hospice and palliative care preferred About the
