Since launching, our team has focused on delivering compassionate, high-quality hospice care in homes, assisted living, and memory care settings across the metro. We provide a full continuum of hospice services including routine care, continuous care, respite, inpatient support, and bereavement, all centered around dignity, comfort, and connection . What You'll Do Provide routine and PRN hospice visits in homes, assisted living, and memory care settings. Deliver pain and symptom management , wound care, and skilled nursing assessments. Support patients and families through end-of-life transitions with empathy and clarity. Communicate confidently about the hospice journey, expectations, and care plans . Collaborate with physicians, intake, chaplains, and the interdisciplinary team . Maintain accurate, timely EMR documentation to support continuity of care. What We're Looking For Active Minnesota Registered Nurse license. Experience in hospice, oncology, palliative care, or end-of-life nursing required. Strong emotional intelligence and ability to support patients and families during critical moments. Confident, compassionate communicator with strong clinical judgment. Comfortable working independently in a field-based role . Reliable transportation and ability to travel throughout the service area.
