Job Description
UpliftedCare is seeking a dedicated and compassionate Nurse Team Leader to supervise the implementation of our patient service program. As a vital member of our Inter-disciplinary Team (IDT), you will share responsibility with the Director of Clinical Services to maintain high-quality hospice care. You will oversee professional and non-professional hospice care providers, IDT members, and contracted personnel to ensure our agency's objectives and policies are met with excellence.
Key Responsibilities Clinical Leadership:
Plan, direct, and participate in the delivery of hospice services. Assume day-to-day clinical operations in the absence of the Director of Clinical Services.Staff Supervision:
Supervise, evaluate, and mentor nursing staff. Assist with interviewing, orientation, corrective actions, and ongoing staff education.Care Coordination:
Serve as the delegated Chair of an Interdisciplinary Team. Guide staff in developing appropriate, individualized care plans for patients and families.Admissions & Referrals:
Review all hospice referrals for appropriateness, ensure initial patient/family contact within 24 hours, and conduct or review initial needs assessments.Compliance & Quality:
Collaborate on Quality Assessment and Performance Improvement (QAPI) projects. Ensure compliance with local, state, and federal regulations.Case Management Support:
Assist with verifying eligibility, obtaining documentation, and negotiating insurance coverage to maximize reimbursement.On-Call & Coverage:
Participate in the administrative on-call rotation (including weekends). Assign staff home visits at the time of a patient's death.Minimum Qualifications Education:
Graduate of an approved school of nursing. Bachelor's degree in Nursing (BSN) required, or on track for completion within 3 years of hire.Licensure:
Current Registered Nurse (RN) license in the state of Illinois.Certifications:
Certified Hospice and Palliative Nurse (CHPN) designation, or successful completion within 3 years of hire. Current CPR certification.Experience:
Minimum of two (2) years of hospice nursing experience required.Transport:
Valid Illinois driver's license, reliable transportation, and active auto liability insurance. Required Skills & Attributes Ability to maintain a professional demeanor under physical and emotional duress. Strong independent judgment, discretion, and multi-tasking skills under pressure. Proficiency with healthcare software and clinical technology. Commitment to maintaining strict patient confidentiality. Collaborative team-player attitude with a focus on fostering a positive workplace culture.Pay:
$85,000.00 - $95,000.00 per yearBenefits:
