• Job Summary:

• We are seeking a dedicated Registered Nurse Case Manager to join our healthcare team. The ideal candidate will provide comprehensive end-of-life nursing care and case management services to patients in need.

• Responsibilities:

• Conduct patient assessments and develop individualized care plans
• Coordinate patient care activities with healthcare providers
• Monitor and evaluate patient progress
• Provide patient education on health conditions and treatments
• Collaborate with interdisciplinary teams to ensure optimal patient outcomes
• Utilize medical records for accurate documentation and reporting

• Experience:

• The successful candidate should possess the following skills and qualifications:
• Active Registered Nurse license
• Willing to travel
