Hospice RN Case Manager- Carrollton, TX

Job Overview We are looking to hire a full-time RN Case Manager to join our hospice team serving patients near Carrollton, TX and throughout the Dallas and Collin County areas of Texas. Our nurses provide compassionate, patient-centered care to hospice patients in the comfort of their homes and care facilities. A strong candidate has experience in hospice, is kind and responsive to emerging patient needs, and maintains compliance with all state and federal regulations and required licensure Responsibilities Conduct initial and ongoing comprehensive assessments of hospice patients and families Develop, implement, and update individualized plans of care in collaboration with the interdisciplinary team Serve as the primary point of contact for patients, families, and caregivers throughout the hospice journey Manage pain and symptom control; communicate changes in condition to the physician and IDT promptly Supervise and coordinate care provided by LVNs, aides, and other team members Provide education and emotional support to patients and families regarding end-of-life care Document all assessments, visits, and communications accurately and timely in Kinnser Consolo EMR Participate in IDT meetings and contribute to care plan updates Requirements Prior experience in hospice care is required Proficiency with Kinnser Consolo EMR system Ability to perform clinical skills such as wound care, catheter care, sterile technique, and tube feeding management accurately. Valid RN license in the state of practice with current CPR/BLS certification Valid Texas driver's license and reliable transportation Benefits Competitive salary Mileage reimbursement Health, dental, and vision insurance No On-Call required RN Salary•$80,000•85,000 per year
