Hospice Social Worker Helena, MT 59604 $29.23 - $45.38 an hour - Full-time $29.23 - $45.38 an hour - Full-time The Staff Pad is proud to partner with a reputed hospital in Helena, MT and we're looking for a compassionate and dedicated Hospice Medical Social Worker (MSW) to join an interdisciplinary hospice team and provide exceptional emotional, psychosocial, and community-based support to patients facing chronic or terminal illness. In this role, you'll work alongside physicians, nurses, (physical, occupational & speech) therapists, chaplains, and volunteers to help patients and families navigate the hospice journey with dignity, comfort, and compassion. You'll serve as a trusted advocate, counselor, and resource liaison while helping create individualized plans of care that support the person — emotionally, socially, spiritually, and practically. This is an ideal opportunity for someone who combines strong clinical judgment with empathy, advocacy, and a commitment to patient-centered care. What You'll Do Provide compassionate guidance, advocacy, counseling and emotional support to patients and families throughout the hospice journey, helping them navigate the healthcare system and connect with essential resources. Conduct comprehensive bio-psychosocial assessments & ongoing reassessments to guide individualized care planning Partner closely with the interdisciplinary hospice team during daily huddles and IDG meetings to provide meaningful psychosocial, emotional, spiritual, and financial insights that help guide individualized patient care Offer emotional support, counseling, education, coping strategies and resources for patients and loved ones facing chronic or terminal illness Assist families with advance care planning, including advance directives, POLST forms, guardianship, and medical power of attorney Help patients and families access financial assistance programs, disability benefits and insurance resources including VA, SSI, Medicare, Medicaid and Private Insurance options Connect patients and families with local resources such as transportation, respite care, meal assistance, grief support, and long-term care placement options. Support bereavement and grief counseling needs while coordinating with chaplains (for faith-based support), medical facilities (for DME and travel needs) and community partners. Advocate patient needs and help families navigate healthcare systems and care decisions with confidence and compassion Serve as a community resource liaison and advocate, connecting patients and families with local, state, and federal programs and services to support their ongoing needs. Educate and guide patients and families in understanding medical information, treatment plans, and available care options to support informed decision-making Set meaningful goals, ensure adherence to care plans, and address barriers that may impact appropriate treatment Maintain compliance with hospice philosophy, documentation standards, and professional social work practices. What We're Looking For Master's degree in Social Work (MSW) from a program accredited by the Council on Social Work Education (CSWE). Current Montana social work license (LSW or LCSW) or actively working toward licensure. Minimum of one year of social work experience in a healthcare setting required Hospice, home health, palliative care, or community-based care experience preferred. Knowledge of chronic disease management, end-of-life care, and psychosocial support services preferred. Strong communication, counseling, advocacy, and care coordination skills Valid driver's license and ability to travel within the service area Why Join Our Hospice Team?
