Job Description

Hospice of the Calumet Area is looking for a dynamic Full-Time Medical Social Worker who is passionate about quality end-of-life care and our Cherishing Life philosophy. Responsibilities include Conduct comprehensive psychosocial assessments to determine patient and family needs and develop appropriate treatment plans. Engage in case management activities, ensuring clients receive the necessary resources and support services. Collaborate with hospice interdisciplinary team to coordinate patient/family care. Provide education and support for patient/family on coping strategies related to terminality, anticipatory grief and death. Maintain accurate medical documentation in compliance with HIPAA regulations, including patient records, notes, care plans and advance directives. Qualifications The successful candidate will possess: Master's Degree from an accredited school of Social Work. Two years of experience in health care; hospice or home care preferred. IN/IL Clinical Social Work License.
