Active unrestricted VA Medical License and DEA w/ VA address & schedules 2,2N,3,3N,4 & 5

Board Certification:

American Board of Internal Medicine (ABIM), American Board of Family Medicine (ABFM), and American Board of Medical Specialties(ABMS).

Merit Badge:

ACLSImmunizations:

MMR/Varicella (2) Vaccines or titers with values indicating immunity for MMR & Varicella. TB (past 12 months)or a QuantiFERON Gold indicating immunity within three months. 2nd TB test to be completed prior to activation and privileges given.
