Chinle Comprehensive Health Care Facility in

Chinle, AZ Start:

Late July Schedule:

12 hrs/shift, 84 hrs/week, 7 days on 7 days off

Shift:

8a-8p hospitalist; 8p-8a nocturnist

Contract Length:

3 months with option to extend up to 24 months

Pay Rate:

$250/hr +

Travel and Lodging Requirements:

Board Certified in Internal medicine, Family Practice, or Med/Peds (ABIM, ABFM, or ABP) New grads may be board-eligible only Completed US residency - 36 months for FP or IM, or 48 months for IM/Peds Minimum 6 months of Hospitalist experience No lapse in clinical practice greater than 6 months No prior loss or suspension of licensure Active, unrestricted state medical license Active, unrestricted DEA with schedules II-V Current BLS and ACLS. Med/Peds candidates also need PALS and NRP Clean NPDB Willing and able to commit to 7 on 7 off for at least 4 consecutive weeks All malpractice suits and board actions disclosed up front 3 current letters of reference (one from Chief of Staff/Medical Director/Immediate supervisor from the hospital where candidate currently or most recently held privileges. New grads- one letter must be from the program director or service chief.
