Role:
Join our dedicated team at Bastrop County Emergency Food Pantry & Support Center Inc as a Community Health Worker for our Maternal Health & Food Security Program. This role is vital in supporting families in need, ensuring access to essential health resources and nutritious food. Responsibilities:
Conduct outreach to expectant mothers and families to assess health and food security needs. Facilitate access to maternal health services and nutrition programs. Provide education on healthy eating, prenatal care, and available resources. Collaborate with local healthcare providers and community organizations. Document client interactions and maintain accurate records of services provided. Organize and participate in community health events and workshops. Advocate for clients to ensure they receive necessary support and resources. Monitor and evaluate program effectiveness and client satisfaction.
Requirements:
Experience in community health, social work, or a related field. Knowledge of maternal health issues and food security challenges. Strong communication and interpersonal skills. Ability to work independently and as part of a team. Proficiency in record-keeping and reporting. Compassionate and empathetic demeanor towards clients. Valid driver's license and reliable transportation. Fluency in English; bilingual abilities are a plus.
