This role offers an exciting opportunity to provide personalized, high-quality home and community care, ensuring clients feel safe, comfortable, and supported in their daily routines. As a vital member of our caregiving team, you will deliver tailored assistance that promotes independence and enhances well-being, all while building trusting relationships with clients and their families. This position emphasizes professionalism, empathy, and proactive care in a dynamic environment that values your expertise. Responsibilities Deliver comprehensive personal care by assisting with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and mobility support using tools like Hoyer lifts when necessary. Monitor patient health through vital signs assessment, medication administration, catheter care, and managing hospice or dementia-related needs with sensitivity and precision. Observe patients closely for changes in condition, documenting medical records accurately using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems while maintaining strict compliance with HIPAA regulations. Support clients with meal preparation, light cleaning, laundry, and home organization to promote a safe and comfortable living environment. Assist with specialized care such as Alzheimer's care, memory care, working with individuals with developmental disabilities or disabilities, and providing hospice support. Utilize medical terminology appropriately when documenting patient information or communicating with healthcare teams. Employ CPR certification skills during emergencies while adhering to safety protocols for patient and caregiver well-being. Skills Proven experience in home health or senior care environments, including assisted living or long-term care settings. Strong knowledge of patient observation techniques and medical documentation practices. Familiarity with EMR/EHR systems for accurate record keeping and communication. Ability to assist with complex medical procedures such as catheter care, medication administration, and vital signs monitoring. Excellent caregiving skills for patients with dementia or neurological conditions like Alzheimer's disease; experience in memory care is highly valued. Working knowledge of medical terminology relevant to home health nursing and caregiving roles. Competence in operating mobility aids such as Hoyer lifts safely and effectively. Experience working with individuals with disabilities or developmental challenges in community or residential settings. Ability to perform light housekeeping tasks including cleaning, laundry, cooking, and maintaining a hygienic environment. Strong interpersonal skills to build trusting relationships with clients and their families while respecting privacy under HIPAA guidelines.
