Job Description

Nurse Practitioner Opportunity - Concierge Internal Medicine Practice I am an internal medicine physician seeking a dedicated nurse practitioner to join a growing concierge-style internal medicine practice focused on providing personalized, high-quality care to adult male and female patients. This role offers the opportunity to develop meaningful, long-term relationships with patients while working in a small, collaborative, team-oriented environment. The nurse practitioner will co-manage an established and expanding patient panel, providing comprehensive outpatient care with an emphasis on prevention, wellness, chronic disease management, and evidence-based medicine. Our practice model prioritizes accessibility, continuity, and proactive healthcare. Patients are typically seen every three months for routine follow-up and are accommodated for same-day or next-day visits when urgent needs arise. The NP will have the opportunity to spend quality time with patients, participate in individualized care planning, and help deliver a higher level of personalized medical care than is possible in traditional high-volume settings.
Responsibilities include:
Co-managing a panel of medically complex adult patients Providing preventive care and chronic disease management Supporting proactive health monitoring and patient education Collaborating closely with the physician and healthcare team Participating in limited night and weekend call responsibilities Typical office hours are Monday-Friday, 8:00 AM-3:00 PM. Ideal candidates include nurse practitioners with: At least 3 years of experience caring for medically complex patients in an outpatient or inpatient setting Strong knowledge of preventive care guidelines and evidence-based practice Excellent communication skills and a commitment to patient-centered care A desire to build lasting relationships with patients and contribute to a collaborative practice environment Newly licensed nurse practitioners with significant prior registered nurse experience and a strong clinical foundation will also be considered. The ideal candidate is someone who values thoughtful, relationship-based care and is passionate about improving the healthcare experience through personalized, accessible, and comprehensive medicine.
Pay:
$100,000.00 - $120,000.00 per year
