Full-time/Exempt, Salaried The Flexible Housing Pool (FHP) Case Manager QMHP provides comprehensive, individualized support services using a client centered harm reduction, trauma informed model for clients in the Flexible Housing Pool. Responsibilities include bi-monthly home visits to clients living in scattered site units across the Chicagoland area, accompanying clients to medical & benefits appointments and connecting clients to community-based resources. The FHP Case Manager QMHP provides a range of professional behavioral health services to individuals impacted by severe mental illness, mental health, and substance use conditions. This role is responsible for all aspects of providing immediate crisis support, supporting in mental health and substance use disorder evaluations, establishing referrals for the appropriate behavioral health level of care, monitoring progress of clients and ensuring continuity of care is not compromised. The Case Manager QMHP will provide a range of clinical assessments, interventions, and engage in internal and external staffings. The Case Manager QMHP will be provided extensive training to assess using IM+CANS to provide clinical recommendations to improve quality of life to returning residents.
Case Management:
Provide case management services including intakes, assessments, service planning, referral and linkage management, and advocacy for a caseload of individuals or families who were formerly experiencing homeless. Identify client needs, including barriers to receiving appropriate services and assist with effective problem solving to address issues. Conduct bi-monthly home visits to ensure housing stability, support in development of life skills, and foster emotional support. In addition to home visits, case managers may meet clients in the community and provide additional support through phone calls and coordination with other team members. Perform crisis prevention and interventions as needed using Harm Reduction & Trauma informed Philosophies. Act as a liaison between landlords/property managers and clients. Work with the landlords to resolve landlord/clients disputes. Assist clients in meeting all lease requirements including documentation and income certification. Work with clients to assist them in addressing issues in order to prevent eviction. Coordinate transition from bridge units, move-ins to permanent supportive housing and lease renewals. Coordinate moving, furniture, and starter kit resources with the FHP team members Assist clients with rehousing should they require a move before their lease is up and/or at the time their lease is up. This includes completing housing lease renewal paperwork and submitting it to Flexible Housing Pool Payment Processing Department before deadlines to ensure continued housing stability. Ensure clients are receiving all necessary covered health insurance benefits including medical and behavioral health services. Ensure clients respond to requests to verify eligibility as part of the State's Medicaid redetermination process in a timely manner so as not to lose medical coverage. Cover for a rotating shift on the 24/7 crisis line available to FHP clients. Make appropriate referrals for services based on client goals and needs and ensure that linkages are secure and functional. Provide education and resources based on the needs of client with regard to maintaining housing and improving income status, community involvement, personal hygiene, life skills, and other identified needs. Assist clients in enrolling in public benefits such as SNAP, SOAR or employment readiness programs. Assist clients in obtaining state ID, social security card, birth certificate, and other necessary documents to receive services. Verify enrollment in medical care or support clients to engage in medical care by providing information, assisting in finding medical provider or helping clients make appointments Refer and link clients to appropriate services within the system of care that promotes positive health outcomes, treatment adherence, and greater self-sufficiency. Qualified Mental Health Professional (QMHP)
Responsibilities:
Provide mental health screenings and ongoing clinical support rooted in trauma-informed care. Develop and implement individualized treatment interventions for clients with identified mental health needs. Collaborate with interdisciplinary teams to ensure continuity of behavioral health services. Utilize clinical judgment to assess risk, provide crisis intervention, and determine appropriate levels of care. Maintain compliance with Illinois Department of Human Services (IDHS) Rule 132 and Rule 140 guidelines for QMHP practice. IM+
CANS Assessment Responsibilities:
Complete Integrated Assessment and Treatment Plan (IM+CANS) in accordance with Illinois Medicaid requirements. Conduct comprehensive bio-psychosocial assessments to determine clinical needs and strengths. Update IM+CANS every 90 days or as clinically indicated based on client progress or significant life changes Collaborate with clients to set measurable, person-centered treatment goals. Document all assessment findings and treatment plans in compliance with billing and accreditation standards.
Member of FHP Partnership:
Work in collaboration with the Flexible Housing Pool Systems Integration Team (SIT) to receive referrals for additional services, attend all SIT meetings to provide updates, address client concerns and successes, and troubleshoot issues with peers. Attends all mandated Flexible Housing Pool training. Maintain appropriate and timely communication with Flexible Housing Pool team, partner agencies, Care Coordinators, and hospital partners to ensure high quality services are provided to the client. Maintain the confidentiality of client information in any written, oral, or electronic form including, but not limited to, health information, HIV/AIDS status, and disability information. Release such information only as needed and with the proper release from the client unless mandated by law.
Documentation & Recordkeeping:
Complete case notes & services topics in CaseWorthy and EHR systems within five-business day of interaction. Complete annual & six-month assessments and enter in CaseWorthy within five business days of completion. Enter all referrals in CaseWorthy Maintains all applicable and required documentation in participants' files Work collaboratively with the Flexible Housing Pool data teamto ensure data quality. Complete all Flexible Housing Pool paperwork and additional agency paperwork when necessary. Complete self-reported health assessments and other tools requested by funder or Flexible Housing Pool team. Attention to Detail Autonomy Administrative Ability Knowledge of the Affordable Care Act (ACA) Dependability Ability to work with the homeless populations Planning and Organizing Supportive Housing Communication Verbal & Written Required Qualifications
  • Master's degree in social work, counseling, or another human service-related field required
  • Must meet Illinois state requirements for Qualified Mental Health Professional (QMHP) credential
  • Must have a valid driver's license and access to a vehicle
  • Experience completing IM+CANS assessments within Medicaid-funded behavioral health settings
  • Ability to conduct clinical assessments, formulate diagnoses, and develop treatment plans under Medicaid guidelines
  • Strong understanding of trauma-informed care, harm reduction, and housing-first principles
  • Demonstrated ability to work with individuals experiencing homelessness, mental health conditions, and co-occurring disorders The ability to safely and successfully perform the essential job functions consistent with the ADA and other federal state, and local standards The ability to stand or sit for long periods of time The ability to lift 20-25 lbs.
