Job Description

Memory Care Certified Activity Aide Rate:
$21.00 - $23.00/hr. (depending on experience)
Sign-on Bonus:
Up to $3,000

Full-Time Schedule:
11:00 AM - 7:00 PM including every other weekend Sunset Nursing and Rehabilitation Center is looking to add a Memory Care Certified Activity Aide to its team. In this role, you'll support meaningful and engaging activities designed specifically for residents with dementia and memory impairments while helping create a safe, compassionate, and enriching environment. RESPONSIBILITIES
Assist with planning, organizing, and leading memory care activity programs in collaboration with the Activities Director Engage residents in therapeutic group and individual activities designed for those with dementia-related conditions Transport and assist residents to activities, meals, and visitations according to care plans Maintain activity attendance records and assist with monthly activity scheduling Set up, clean up, and maintain activity equipment and supplies in safe condition Assist with resident care needs, dining room preparation, and redirection of residents as needed Participate in QOL meetings, CQI initiatives, and required in-services Maintain HIPAA, infection control, safety, resident rights, and corporate compliance standards Support residents, families, visitors, and team members with professionalism and compassion Perform additional duties as assigned by supervisor
REQUIREMENTS
Valid NYS Certified Nursing Assistant (CNA) certification required High School diploma required; Associate degree preferred Minimum of 6 months CNA experience working with dementia population required Strong communication, teamwork, and customer service skills Ability to lead activities independently and work collaboratively with interdisciplinary teams Compassionate, patient, and resident-centered approach to care Dependable, professional, and able to maintain confidentiality and safety standards Ability to work scheduled evenings and weekends as needed
