Monday-Friday 8AM-4PM House Manager is on call for emergencies and emergency shift coverage. General Summary Responsible for providing direct personal care and support with dignity and respect in a manner that meet the health, social, emotional, safety and physical needs of people supported in accordance with their PCSPs and the general policies and procedures of DDA. Providing direct assistance with activities of daily living (ADLS) such as bathing, dressing, meal preparation, feeding eating, house keeping, laundry, and community participation/ outings is an essential function of this job. Assisting People Supported to acquire physical, intellectual emotional and social skills needed to live and enjoy life independently in a non-institutional setting is a critical in this job. Assist Program Coordinators with managing the provision of supported living, personal assistance, individual transportation and community based day services in an environment designed to meet the social, emotional, safety, health and physical needs of people supported in accordance with their PCSPs and the general policies and procedures of DDA. Take active role to coordinate staff scheduling, training and performance evaluations in a manner that promotes staff motivation and staff retention. Monitors service documentation and records of the program. Ensure compliance of operations with DDA policies and procedures. Essential Job Functions Provide assistance as necessary with ADLS such as bathing, dressing, laundry, meal preparation, feeding, housekeeping, and leisure activities as specified in the PCSP. Assist Person Supported to learn, acquire and develop physical, intellectual, emotional, and social skills needed to live independently and enjoy life as specified in the PCSP. Provide personal care and support with dignity and respect in a manner that meets the social, safety and health needs of Person Supported Assist Person Supported to implement goals and outcomes as specified in the PCSP Document in shift notes the level of assistance provided (total, partial/maximum assistance) Document in shift notes Person Supported's reaction and progress towards goals and outcomes set in the PCSP. Write complete and legible shift notes in a timely manner, specifically before the end of the shift. Promote, advocate and uphold the rights of Person Supported. Follow agency Policies and procedures Provide personal care and supports in an ethical way that meets basic performance standards Speak politely and include Person Supported in your conversation Use positive communication and explain things in a clear and understandable manner. Encourage Person Supported to think and teach them to do much for themselves Consider Person Supported's feelings and respect individual differences Listen to Person Supported's point of view and assist them to make decisions and choices. Give medication accurately and timely when authorized to do so by your supervisor Complete and sign timesheet in a timely manner and record the actual hours you worked. Attend all required training as per DDA requirements, failure to attend will result in disciplinary actions. Protect Person Supported from harm by reporting abuse, neglect and exploitation in accordance with DDA's Protection From Harm policy. Report all incidents to your supervisor and complete accurately, and timely Reportable Incident Form Attend meetings and circle of support meetings as required by your supervisor Transport Person Supported using road worthy own vehicle (mileage will be reimbursed) or vehicle to accomplish community participation and social integration goals as per PCSP. Provide personal care and meet health and safety needs of service needs by adhering to universal precaution standards. Read, understand and implement the PCSP. Ensure that all approved supported living, personal assistance, individual transportation and community based day service hours are provided as specified in each service recipient's PCSP by all staff Create an environment that allows the delivery of supports services to meet social, health, safety and other personal needs of people supported as defined in their PCSP. Supervise direct care staff, house managers and companions in a manner that motivates and promotes high performance and staff retention. Coordinate staff scheduling with house managers/companions/team leaders to ensure adequate shift coverage and avoid unnecessary over-time work. Train all staff and ensure that staff possesses required and up-to-date training before staff is scheduled to work as per DDA Training Requirements Policy(please note you are responsible for your staff training and you cannot use untrained staff) Ensure orientation is successfully completed by new staff and that staff is acquainted with the philosophy, goals, policies and procedures Evaluate staff performance objectively and recommend appropriate action (i.e. promotion, pay raise, training, discharge) Ensure that staff are writing service notes that support employee hours worked and billing requirements Manage the home in a manner that ensure maximum compliance with all quality assurance domains. Ensure the health and safety of service recipient through scheduling medical appointments and follow-up; ensuring proper administration of medication, adequate provision of food and supplies; and ensuring nutritional needs are met. Maintain and document constant communication with ISCs and parents/guardians and resolve issues presented in a professional and ethical manner that promotes agency public image. Perform supervisory visits and ensure staff documentation adequately reflect on-going implementation of the PCSP. Train staff to document accurately Protect people supported in accordance to protection from harm policy & participate in incident management meetings. Assist people supported to participate in the management of their funds per their PCSP Properly manage and account for all house hold and personal purchases Maintain a daily transaction log of cash purchases and keep receipts for all transactions Maintain a transaction log of food stamp purchases and keep all receipts Ensure food stamp benefits and living expenses/purchases are shared equally among room mates. Update personal property inventory and turn in reports to the office quarterly. Complete Monthly Vehicle Inspections and submit to the Nashville office in a timely manner, ensuring accuracy of VIN number and mileage. Ensure current vehicle paperwork is located in the vehicle at all times, including but not limited to: vehicle registration and vehicle insurance. Perform other duties as assigned. Critical Success Factors- to be successful in this job; accomplish all of the following: Provision of all approved personal assistance/supported living/CBD service hours. Protection from harm of all People Supported. Effective implementation of PCSP. Complete and timely documentation of services. Complete training and retention of assigned staff. Complete knowledge of DDA provider manual policies. Scope of Responsibilities Keep abreast of laws & DDA state policies that apply to Medicaid waiver services and maintain compliance. Decision Making On a regular and continuous basis, exercise judgment and authority to carry out tasks 1-25 above, seek approval of the Program Coordinator/Director on decisions with consequences that impact people, costs, and or quality of service or the image of

Authority Has authorization to direct the activates of assigned direct care staff and activates of the personal assistance program in a designated area or homes. Communication Non- routine exchanges, routine information using tact's and persuasion as appropriate requiring good oral and written communication skills. #IND1
