Shift Supervisor
SUMMARY OF DUTIES
An employee in this position ensures proper resident care is provided by the resident care staff to residents in a Home for Aged (HFA) Assisted Living and Memory Care environment and is responsible for coordinating and monitoring each resident's individualized service care plan. Employees of this job work under the supervision of the Licensed Practical Nurse or Director of Resident Care. They are responsible for the direct supervision of the Resident Care Assistant(s) and Medication Passer(s). Third shift premium.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Direct and monitor resident care provided by the care staff to ensure each resident's individual needs are consistently met and each resident's service care plan is being followed to provide the best possible care and living environment for the resident. Communicating new programs, procedures, interventions and/or changes in a resident's health/circumstance to the care staff, and resident's physician when indicated, including coordinating necessary resident physician appointments.
  • (only contact when LPN indicates or delegates task) Assist resident care staff in completion of their job responsibilities as necessary; lead care staff in gaining exceptional skills in the care of residents; monitor resident care staff work performance; regular monitoring of daily communication log.
Develop and maintain an open relationship with residents of the community through regular interaction, encourage and support residents with their involvement in the community.
  • Function as contact person for resident, resident's family, physician, healthcare agencies providing additional services to the resident, and community's contracted pharmacy regarding resident medications/adjustments.
  • When LPN is not on site at the community. Responsible for coordination of residents' medications including, monitoring of medication passer(s), medication documentation, controlled substance medication count and disposal according to state regulations; communicate with resident, resident's family,
  • physician and pharmacy regarding medication orders and/or changes in resident medications ._ .
  • When LPN is not on site at the community_ Bi weekly inspection of medication cabinets as assigned by LPN .
Serve as the first line for mitigating concerns with the resident and/or resident's family and provide support and information to each resident and their family; respond promptly to a resident's need/concern. Meet with Licensed Practical Nurse and/or Director of Resident Care to determine needs and challenges of resident care, including possible discharge; coordinate with Licensed Practical Nurse and Director of Resident Care to confer with resident and/or resident's family regarding changes in a resident's status. Monitor documentation in and completion of resident records, medication administration record, and physician orders to ensure compliance; conduct resident chart audits to ensure accuracy and completeness of resident's medical record. Responsible for the safety and well being of the residents including completion of resident incident reports in compliance with licensing regulations All reports to go to the LPN for ; responsible for safety of residents in case of emergencies; identify potential safety hazards, initiate corrective action to address potential hazard and inform Licensed Practical Nurse or Director of Resident Care. Oversee completion of regularly scheduled events regarding resident safety/health, e.g. resident vital signs, fire drills, etc.. Maintain clean and organized Medication Rooms, including maintenance of equipment and inventory of resident care supplies including PPE and first aid supplies, etc; report to LPN needed supplies, equipment repairs, etc. Practice appropriate lifting and transfer techniques when assisting residents; awareness of general safety, environmental, and infection control procedures; familiar with (PPE) Personal Protective Equipment and universal precautions relating to OSHA-Blood-borne Pathogen Standard. Knowledge of location of community's Disaster Manual and Material Safety Data Sheets (MSDS) and the reporting system specified in the Right to Know policy. Excellent attendance, timeliness in reporting to work, and discussion of all absences, injuries and illnesses with Licensed Practical Nurse or Director of Resident Care; attendance at professional seminars and community activities. Attend minimum of 10 in-service training programs per 12 month period. Perform other related duties as assigned.
SUPERVISORY RESPONSIBILITIES
Provide direct supervision to resident care staff and medication passer(s); schedule resident care and medication passer(s) for adequate coverage, direct, monitor and motivate staff and staff performance, assess workload, assign work responsibilities, review and monitor care staff time cards. Schedule and assign resident care staff to ensure sufficient but not excessive resident care staff is available to meet the needs of the residents in a cost effective manner. Encourage and provide opportunities for resident care staff to provide input into policies and procedures to ensure quality resident services; meet with staff to determine needs and challenges of job responsibilities. Assist Director of Resident Care and Licensed Practical Nurse in the development and implementation of departmental policies, procedures and operations; accountable, dependable and assertive in identifying departmental and staffing needs. Assess problem situations to identify causes, process relevant information, generate possible solutions and make recommendations to resolve problem. Provide leadership and encourage a professional, positive attitude among community staff by being a role model in conduct, appearance, communication, mutual respect and ethical behavior and decision making. Present an open, positive, and professional image to all residents, visitors, co-workers and general public by displaying general warmth and kindness in a friendly, courteous, respectful manner at all time and under all circumstances. Regular communication with Licensed Practical Nurse and Director of Resident Care (including a daily report identifying staff performance and resident health condition/issues) and provide input into community's resident care program. Assist Director of Resident Care and Licensed Practical Nurse with selection of resident care staff and make recommendations regarding disciplinary action, work performance, salary adjustments, promotions and terminations or resident care staff. Participate in planning and implementation of on-going quality assurance process and improvement efforts consistent with community's philosophy and mission statement.
JOB REQUIRMENTS - KNOWLEDGE, SKILLS, ABILITIES
Ability to perform necessary job responsibilities of Resident Care Assistant and Medication Passer. Friendly, courteous manner; highly motivated, patient; excellent communication skills to direct resident care to range of residents with varying degrees of abilities. Knowledge of assisted living and memory care principles, techniques, methods and current trends/developments regarding resident care including knowledge of Resident's Rights, federal/state licensing and regulations for Home for Aged community. Knowledge of computer PC operations and software e.g. Microsoft Word, Excel, Outlook, etc.; good organizational and prioritization skills. Ability to be flexible, solve problems/conflicts effectively in a sensitive and calm manner, act independently and carry out oral and/or written instructions; ability to react quickly and decisively in the event of an emergency. Affinity for seniors and genuine care regarding their well-being with an ability to maintain appropriate attitude and conduct for the welfare of residents; compassionate; demonstrate an appreciation of the heritage, values, and wisdom of the resident and an understanding of their rights, the aging process and diseases of the elderly. Display honesty, integrity, dependability and ability to work as a team member, possess good leadership and interpersonal skills and maintain a positive attitude toward employment; able to communicate effectively and maintain confidentiality of resident and employee information in compliance with HIPPA regulations. Reliable transportation to work with flexibility to work on any day of the week, any designated shift. Ability to identify own personal strengths and weaknesses and discuss with supervision. Satisfactory references, background check, drug screen, pre-employment physical and current PPD(or chest x-ray) for tuberculosis.
EDUCATION & EXPERIENCE
Associates degree in Business Management, Health Care or Medical Records CNA Certification or minimum 2 years experience in health related and/or elder care environment Previous work experience as a Resident Care Assistant and Medication Passer
PHYSICAL DEMANDS AND WORK ENVIRONMENT
Work is performed in a residential community setting; possible exposure to unpleasant conditions such as cleaning of human waste and bodily fluids to maintain a sanitary environment for residents, visitors and staff of the community. Speaking and hearing required to perform job responsibilities; reading/writing; understand directions and communicate verbally. Specific vision abilities required include color, close and far vision acuity, depth perception and ability to adjust vision focus. Employee must be in good physical health and have the ability to climb, balance, kneel, crouch, reach, walk-at least 3 miles/day, sit, lift, grasp, perform repetitive motions involving extensive bending/twisting-at least 25 times/day, stooping, pushing, pulling, fingering, wrist motions and foot operations; ability to stand for an extended period of time without significant rest period; possess the strength/stamina to lift more than 100 lbs. with assist.
