7/7 Triage RN Coordinator - HOSPICE Position Overview THEMA Health Services is a well-established agency and has been in business for nearly 30 years. Our hospice is serving patients across Arizona. We are growing and expanding our on-call team so we can continue to provide exceptional care after hours. We are seeking a Triage RN with strong critical thinking skills, organized, and the ability to multitask in a fast paced setting to serve as the central nervous system for our field operations during after-hours and weekends. This is not a field-based role; rather, you are the primary point of contact and support for our on-call teams and field staff. In this role, you provide a calm, confident, and compassionate phone presence to our patients and families when they need it most. You aren't just "dispatching"—you are lead with a hospice heart, using your clinical judgment to prioritize needs, provide task-based direction, and ensure that our field staff has everything they need to provide the best possible end-of-life care. 7 Days On / 7 Days Off Key Responsibilities Act as the primary liaison for all shifts worked, managing the flow of information between patients, providers, and field staff with a focus on seamless care transitions. Field inbound calls with professionalism and deep empathy, acting as a steady voice for families during their most vulnerable moments. Provide clear, actionable instructions to on-call teams to ensure efficient routing and high-quality care delivery. Utilize your clinical background to assess the urgency of incoming requests, prioritizing field deployments so that every patient receives a timely response. Serve as the "eye in the sky" for field staff, assisting with documentation queries and troubleshooting logistics so they can remain fully focused on the bedside. Maintain accurate records within our systems, ensuring that every interaction and transition of care is documented with 100% accuracy to maintain the integrity of the patient's journey. Prepare on-call reports each morning for handoff to the care teams. Who You Are You can look at ten moving parts and instantly know which one needs your attention first to ensure patient safety and comfort. You are comfortable toggling between multiple software platforms, phone lines, and messaging apps without losing a beat. You translate complex situations into simple, direct instructions for the team while maintaining a "can-do" attitude. While you don't supervise the staff, you view their success as your own. You are there to advocate for the field team and enhance the patient experience. You notice the small discrepancies in documentation or scheduling that others might miss, ensuring compliance and quality at every turn. Qualifications 1-2 years of experience in hospice. Valid, unencumbered RN license in the state of Arizona. Must possess strong clinical judgment and a commitment to providing an exceptional end of life experience to the families we serve. Solid computer skills, including Microsoft Office solutions, digital communication platforms, and experience with EMR software (Homecare HomeBase is a plus!) Exceptional telephonic and written communication skills with the ability to convey empathy over the wire. Ability to remain calm and focused under pressure in a high-compassion environment. Previous experience in an after-hours coordination, dispatch, or triage-heavy role is required.
Note:
This is an office or remote-based coordination position. While it requires the clinical knowledge and heart of a hospice nurse to guide the team, it does not routinely involve hands-on field work or direct supervisory authority over personnel. This is a full time, salary position that pays (7 days on, 7 days off schedule (pays full 80 hours per pay period).
