  • REQUIREMENTS
  • High School Diploma/GED
  • 1-3 years of experience in human services, housing navigation, or case management
  • 6 months experience working with individuals who identify as having mental health conditions, substance use disorders, disabilities, or other long-term barriers to stability
  • Valid driver's license with acceptable driving history
  • Independently manage a caseload and prioritize multiple and varied tasks while meeting deadlines
  • Working knowledge of program eligibility requirements, documentation standards, and funder guidelines
  • Strong organizational skills, written & oral communication skills and attention to detail
  • Sensitivity to individuals from diverse cultures, backgrounds, and lived experiences
  • Proficient with Microsoft Office and Google Workspace
PREFERENCES
  • Prior experience working with individuals and families experiencing poverty and homelessness
  • Bachelor's degree in social work or related field
  • Bilingual (any language)
  • Experience with HMIS and agency/funder data systems
JOB DUTIES
  • Provide intensive, community-based social work services including coordination, advocacy, support, and encouragement using a Housing First framework.
  • Supports individuals and families experiencing homelessness in achieving and maintaining long-term housing stability
  • Case management responsibilities including screening, intake, assessment, housing navigation, landlord engagement, and ongoing supportive services
  • Maintain a working knowledge of program requirements, including eligibility criteria, enrollment standards, documentation expectations, and funder definitions
  • Assist participants with housing search activities including transportation, landlord/property manager communication, and lease-up processes
  • Provide direct landlord advocacy to support housing placement and retention
  • Provide progressive, person-centered, trauma-informed, and strengths-based engagement and assessment, utilizing peer support and mentoring techniques to promote sustained recovery and encourage the development of strong, community-based support systems
  • Coordinate multi-disciplinary service providers (e.g., healthcare, mental health, addictions, employment, and social services) to ensure comprehensive support
  • Provide education& #8239;regarding& #8239;public benefits (WIC, Medicaid, TANF, SNAP, veteran-specific benefits, SSD, SSI, Medicare, etc.)
  • Maintain up-to-date knowledge of community resources related to housing, mental health, addiction recovery, disability services, and income& #8239;support
  • Ensure and document& #8239;participant progress, services offered, and& #8239;required& #8239;documentation for enrollment, service provision, and program exits is completed within established timelines
HOURS:
Full time, Monday through Friday with occasional early morning, evening, or weekend meetings. Flexible work schedule after completing& #8239;90 days& #8239;of training
PAY:
$25.45/hour
