Job Description
This position is responsible for providing professional case management services that support resident stability, family progress, and successful program participation. This position will complete intake, screening, assessment, service planning, referrals, follow-up, documentation, and ongoing resident support. The Case Manager will use strong case management skills to help residents identify needs, set goals, overcome barriers, access community resources, and work toward housing stability, self-sufficiency, and family wellness. The Case Manager will maintain professional and supportive relationships with residents while using appropriate boundaries, sound judgment, and trauma-informed practices. Responsibilities include coordinating referrals and services related to housing, benefits, employment, education, childcare, behavioral health, healthcare, child and family supports, and other community resources. The position will also support resident accountability, program participation, service plan progress, and compliance with housing and program expectations. The Case Manager will complete accurate and timely documentation of services, contacts, referrals, progress, incidents, goals, and other required case activity. Documentation may be used for program reporting, grant reporting, HMIS, Medicaid billing readiness, internal compliance,and quality assurance. The position will follow all confidentiality standards, HIPAA requirements, mandatory reporting laws, data privacy rules, Medicaid documentation expectations, social service ethics, and NLDM policies and procedures. The Case Manager will work closely with NLDM leadership, program staff, property management, community partners, healthcare providers, behavioral health providers, schools, childcare providers, housing partners, and other service organizations. This position requires the ability to manage an assigned caseload, recognize crisis or safety concerns, make appropriate referrals, participate in meetings and trainings, assist with emergency procedures when needed, and perform other duties as assigned to support the mission and operations of New Life Directions Ministries. Required Education and Credentials Bachelor's degree in Social Work, Human Services, Psychology, Counseling, Family Studies, or a related behavioral health/human services field required. Master's degree preferred. Candidate must have strong case management skills and experience with assessment, service planning, documentation, referrals, resident support, housing stability, and community resource coordination. Candidate must be willing and eligible to complete all Medicaid-related training, Targeted Case Management training, documentation training, background checks, and provider requirements needed for NLDM to become approved for Medicaid-billable services. Preferred Credentials Kentucky LCSW, CSW, LPCC, LPCA, LMFT, LMFTA, or other approved behavioral health credential preferred. LCSW is strongly preferred for candidates expected to provide or supervise Medicaid-billable individual therapy, group therapy, behavioral health assessment, or clinical services. Experience with behavioral support services, trauma-informed care, housing supports, family stability services, and Medicaid documentation is strongly preferred.
