Job Requirements
POSITION SUMMARY
Lead Health First's efforts in implementing and evaluating community outreach programs based on identified unmet health needs of the community, working in conjunction with the federally mandated Community Health Needs Assessment, conducted per federal guidelines. Promote wellness and health in the Health First's service area in a positive and community-oriented way. Act as liaison for managing relationships necessary to uphold Health First's accountability and transparency within the community.
PRIMARY ACCOUNTABILITES
Plan, maintain, track and report community benefit activities in accordance with strategic goals and Health First policy for the filing of IRS Form 990, Schedule H. Compile and enter community benefit data into CBISA database. Support the Community Health Improvement Committee and Wellness Grant application, review, and implementation process. Serve as a resource to community partners, maintain program data including outcome reports as well as recommendations for improvement plans, if applicable. Assist in the development, planning, execution, and analysis of the Community Health Needs Assessment (CHNA) to meet regulatory requirements, organizational goals, and objectives. Manage sponsorship requests in alignment with strategic goals, coordinate logistics, and conduct sponsorship committee meetings. Monitor and analyze social determinants of health data and collaborate with internal and external stakeholders to develop initiatives that promote health equity. Serve as subject matter expert on community benefit planning and reporting in collaboration with the Finance Department regarding compliance with federal and state reporting requirements. Monitor Community Benefit budget on a monthly basis by comparing and analyzing actual results with plans and forecasts. Process agreements, invoices, and follow-up for community benefit-related Health First partnerships, sponsorships, and programs. Collaborate with Director, Community Engagement, on communications with external and internal partners to include Health First wellness initiatives, volunteer opportunities, support groups and community benefit programs. Represent Health First at meetings and events that support Health First's mission and community benefit objectives. Coordinate event logistics as needed. Work Experience

PHYSICAL REQUIREMENTS
Majority of time involves sitting or standing; occasional walking, bending, stooping. Long periods of computer time or at workstation. Light work that may include lifting or moving objects up to 20 pounds with or without assistance. May be exposed to inside environments with varied temperatures, air quality, lighting and/or low to moderate noise. Communicating with others to exchange information. Visual acuity and hand-eye coordination to perform tasks. Workspace may vary from open to confined. May require travel to various facilities within and beyond county perimeter; may require use of personal vehicle. Benefits
