Job Description

Cornerstone - Housing Specialist Assertive Community Treatment (ACT) Cornerstone empowers people living with mental health and co-occurring substance use disorders to live, work, and integrate successfully into the community. We envision that each person we serve will have a meaningful life of their choosing. W e offer years of evidence based treatment experience to our consumers and practical training to our staff. Our robust training program allows staff members to develop and grow in the position and provides the basis for upward mobility within the agency. Responsibilities of the Housing Specialist with the ACT Team Supports clients with finding and maintaining safe and affordable housing Provides ongoing assistance to help clients apply for, understand and manage their benefits Ensure maximum acquisition of entitlements and other forms of assistance for all clients served by the ACT team Provide operational administrative support to the ACT team. Assists clients in maintaining employment by frequent visits to the work site Qualifications of the Housing Specialist with the ACT Team Bachelor's degree and one (1) year of case management experience preferred Experience working with the population of serious mental illness in the capacity of case management, integrated care coordination, and medical office organization strongly preferred Driver's license, proof of auto insurance, and access to a vehicle for work purposes. Transporting clients in personal vehicle is required. Eligible for health care (single and family) the 1st of the month following 30 days of employment. Must work a minimum of 20 hours per week to be eligible for benefits and leave accruals. 403(b) retirement plan with employer match available. This is a full-time (40 hours/week) position.
