Job Description

Columbine Caregivers is a locally owned home healthcare agency that has served the Grand Junction area for over 30 years. We pride ourselves in having the best team of care professionals in the valley. Currently we are looking for an outstanding individual to fulfill the role of Care Coordinator - Onboarding Specialist. This position is a full-time position Monday through Friday with some on-call rotation responsibilities. Office hours are 8:30a-4:00p but often times staying past 4:00p is necessary to finish working through time-sensitive matters. The Care Coordinator position is a supervisory role that is primarily office based but does require occasional time spent with clients and/or caregivers in the field. Care Coordinators work both with clients and caregivers to ensure all aspects of care are managed appropriately. This Care Coordinator role will specifically be responsible for staffing and employee relations. Primary duties will include screening candidates, interviewing, on-boarding new employees, creating personnel files, maintaining employment ads, caregiver supervisory visits, and working closely with scheduling to ensure appropriate staffing speed is maintained for current scheduling needs. Other duties include; general phone support, entries into EMR system, helping with new client intakes, updating client service plans, assisting with scheduling, helping with general office tasks, and maintaining office cleanliness. All supervisors participate in an on-call rotation. Must be comfortable with after hours and weekend on-call duties. Most on-call duties are carried out over the phone, however as a last resort supervisors do cover shifts to ensure clients have coverage for every shift. On-call frequency is typically one week per month and one weekend per month but can differ. Must be comfortable covering caregiver shifts. Supervisors do help with shift coverage at times, both during office hours and after hours. The ideal candidate will have home healthcare experience either providing hands-on care or working in a home healthcare office setting. Other preferred experience includes working in a fast-paced office, working in human resources, experience with on-call responsibilities, working in a supervisory role, or other medical office or healthcare related experience. CNA license and/or MA certification is also preferred, but not a requirement. Candidate will be required to pass an extensive criminal background check as well as drug testing prior to employment. At this time, we do not offer full medical benefits, however we do offer Appleton Clinic membership. The company does provide a company laptop as well as bonus pay while performing on-call duties.
